If we look critically at how much heart is visible to the RIGHT of the midline vs to the LEFT, we can see that in our patient (Image A), there is too much heart to the right and not enough to the left, compared to the normal patient in Image B. This could be because the heart is being PULLED to the right (like when part of the right lung collapses) or because the heart is being PUSHED to the right (by increased pressure on the left side). Which do you think is going on in this patient?
