This is an abnormal testis (label says 'SAG LT') with a normal head of the epididymis at the upper end. There is a rounded mass with slightly heterogeneous echo texture in the testis that is worrisome for carcinoma. You can tell that this is a small parts scan with a high frequency transducer because of the square shape of the field. Most masses in the testis are malignant and require removal.
