This is an abnormal uterus (label says 'Sag Uterus') in the sagittal with a normal thin endometrial stripe most consistent with the early part of the menstrual cycle. You can tell that it is a transvaginal scan because the sector is wide. The fundus is projecting toward the head, indicating anteverted position. There is a large fibroid projecting posteriorly and distorting the shape of the uterus.
