This CT image is displayed in a bone window, and there is a skull fracture, circled in red. A skull fracture can injure adjacent blood vessels, leading to this type of hemorrhage.

Labels off middle meningeal
epidural

In an epidural hematoma, the blood is often coming from the middle meningeal artery, which runs along the inner surface of the skull. The blood must peel the dura off the bone, which is firmly attached, so the collection tends to form a lens shape.
