Case 4: 87 year old woman with shortness of breath

There is a large superior mediastinal mass (shown in red) that is producing significant narrowing of the upper trachea, at the tips of the yellow arrows.  One of the commonest causes of a mass in this location is a markedly enlarged thyroid gland, or goiter. Her specific symptom is STRIDOR, indicating upper airway obstruction.
